Steriliser B out of service
Steriliser B is out of service after a cycle failed to hold temperature on load 26-0911. The service technician is booked for 16 September. All loads go through steriliser A until further notice, so plan instrument turnover with the reprocessing coordinator and do not delay transport of used instruments. No packs from the failed load were released.

Two-identifier check at every counter
A pathology result was handed to the wrong patient with the same surname this week and returned unopened. From today every staff member must ask the patient to state their full name and date of birth before handing over any document, result or prescription, and match both to the record. Shared name alerts have been added. Please re-read the identification procedure.

Medical emergency drill, Warners Bay
The annual medical emergency drill at Warners Bay is on Thursday 17 September at 1:00 pm. All staff on site take part in their plan roles. Patients in the building at the time will be told a drill is in progress. Toronto and Charlestown drills follow in October. Attendance is recorded in the credential register.

Influenza and pertussis boosters for staff
The nursing team is offering seasonal influenza and pertussis boosters to all staff during the week of 21 September. Bring your immunisation record. Staff whose records in the credential register are incomplete have been contacted by the Practice Manager. Vaccination is recorded in the register on the day.

Accreditation survey 3 December
The accreditation surveyors will visit all three sites on 3 December. The working group meets fortnightly from 16 September and evidence for each standard is being assembled in the governance folder. Every department lead is asked to check that their procedures are current, their registers are complete and their staff know where to find the policies.
