Audit infection control and emergency planning for accreditation
You are the Quality and Accreditation Coordinator reporting to the Practice Manager.
What has happened
The accreditation survey is confirmed for 3 December and the Practice Manager wants the infection prevention and control evidence and the emergency planning evidence complete, with gaps fixed, by 6 November. Since March the incident register holds a failed steriliser load, a dental assistant sharps injury, a vaccine fridge excursion, a rejected unlabelled specimen and a privacy breach. Steriliser B has been out of service for two weeks. The credential register has staff with resuscitation and hand hygiene training near expiry. The Medical Emergency and Emergency Response Plan was updated this month and the drill is next week. You will present your findings and an action plan to the clinical governance committee on 14 October.

Reprocessing and Steriliser Load Log
Tracks every steriliser load from packaging to release, with cycle parameters, indicator results, the operator and the release decision, so instruments can be traced to patients.
Clinical Incident and Hazard Register
Records clinical incidents, near misses, medication and specimen errors, staff injuries, privacy breaches and hazards with a severity rating, review status and actions.
Pharmacy Stock, Imprest and Cold Chain
Manages pharmaceutical stock levels, imprest supply to the treatment rooms and dental clinic, expiry tracking, scheduled medicine control and fridge monitoring.
Patient Records and Appointments
The appointment book and patient register for all sites, recording bookings, arrivals, clinicians, consultation type and follow-up actions.

Evidence guide
The audit must be evidence based, citing register entries and load numbers, and must not rely on assertion. The theme analysis must connect incidents to system causes and check whether actions were closed. The action plan must have realistic owners and dates before the deadline. The minutes must follow the template, refer to incidents by register number only and record decisions. Look for a clear recommendation on steriliser B.
